A 39 year old male HUMAN patient presents to a HUMAN physician medical doctor MD for a 19 day history of lethargy weakness vomiting and loose stools. The physician performs a physical exam PE. The physician orders requests fills out a laboratory requisition form and checks off the boxes on the paper or electronic request form for the tests that he the physician wants to have the lab run perform to see what’s going on based on the history the patient told him and based on what the physician doctor MD found on the PE like were the patients lymph nodes enlarged YES or NO and then the physician asks himself as the MD doctor physician in charge what disease does he think the patient has? The history and the physical exam findings help the MD human doctor figure out which sicknesses diseases could be are likely to be be plaguing the 39 year human male that is sick and in the hospital or clinic. The physician MD human doctor IN THIS SCENARIO case is concerned mostly about human immunodeficiency virus HIV infection in this patient but SALMONELLA GASTROENTERITIS and SHIGELLA GASTROENTERITIS are also possibilities high up on the list of likelihood and the ranked in order possibility list with most likely disease / most likely to be causing the disease at the top of the list etiology etx style and in descending order from there and this ranked order possibility list is what we sometimes or often call the differential list also known as aka a DDX LIST and this list is made by the human physician MD doctor BEFORE the tests are RUN PERFORMED. THIS DIFFERENTIAL LIST ranked in order most likely to least likely possiblity list is used as the BASIS OF HOW TO KNOW WHAT TO TEST FOR in the way of DIAGNOSTIC TESTS. The patient gets the lab request requisition and GOES TO THE MEDICAL LAB and has BLOOD DRAWN and maybe also gives a URINE SAMPLE and FECAL FECES sample too if the physician was prudent enough and the MEDICAL LAB RUNS THE TESTS and there is a REPORT generated about how the TESTS CAME BACK CAME OUT. Did the HIV Human Immunodeficiency VIRUS test say POSITIVE or NEGATIVE? If the test comes back POSITIVE the physician MD doctor is notifed and this sets off a chain reaction of things but importantly communication with the 39 year old human male patient on things like what the disease HIV is and risk factors and contact tracing and how to reduce risk of transmission. THE PHYSICIAN and any other HEALTH CARE PROVIDERS like NURSES MUST document write down in the chart medical records that they spoke with the patient and asked the questions they needed to ask and told the patient about follow-up needed and these are things they actually did do. The PHYSICIAN Medical human Doctor MD writes down in the human person patient chart that this individual 39 year old male patient tested POSITIVE for HUMAN IMMUNODEFICIENCY VIRUS and the DIAGNOSIS Dx written IN THAT SECTION OF THE CHART READS:
acute human immunodeficiency virus infection
^^^that is the DIAGNOSIS and we abbreivate the word diagnosis as dx or DX
If the test comes back negative and the LAB TEST REPORT says NEGATIVE then the report goes back to the physician human MD doctor health care provider still too and the physician human MD doctor has to talk to the patient still too more and they all go down the road of trying to rule in or rule out other diseases ailments sicknesses and the human doctor MD physician orders requests other diagnostic tests like maybe abdominal x rays or abdominal ultrasound. The process repeats until a diagnosis is made because the patient has SYMPTOMS.
If an HIV test comes back POSITIVE for a patient who is CLINICALLY ASYMPTOMATIC for example in a patient who is first identified via contact tracing then the diagnosis STILL is:

With regards to any approved, commercially available antibody-based (serological) assay for Human Immunodeficiency Virus (HIV) – 1 or HIV – 2, could you please so advise as to which laboratory test is considered acceptable as a “gold-standard” for the purposes of calculating sensitivity and specificity given that there are no [listed] FDA-approved HIV-2 assays (tests) based on nucleic acid test (NAT) methods, per the Complete List of Donor Screening Assays for Infectious Agents and HIV Diagnostic Assays?
Further, given that:
“Human Immunodeficiency Virus (HIV) serological diagnostic and supplemental tests are prescription devices for the qualitative detection of HIV antigen(s) and/or the detection of antibodies against HIV in human fluids or tissues. The tests are intended for use as an aid in the diagnosis of infection with HIV and are for professional use only”,
and that:
“analytical sensitivity of [such] test[s] must be the same as or better than that of other cleared or approved tests[.]”
Per Sections 866.3956, for antigen-antibody based serological tests:
“Clinical sensitivity of the test must have a lower bound of the 95 percent confidence interval of greater than or equal to 99 percent.
[and]
Clinical specificity of the test must have a lower bound of the 95 percent confidence interval of greater than or equal to 99 percent.”
Per Sections 866.3957 for nucleic acid based serological tests:
“Clinical sensitivity of the test must have a lower bound of the 95 percent confidence interval of greater than or equal to 99 percent.
[and]
Clinical specificity of the test must have a lower bound of the 95 percent confidence interval of greater than or equal to 99 percent.”
Reference is now being made to the following “FDA approved” test:
